Lodwick-Madewell Classification (Lytic Bone Lesion)

How this tool works

Lodwick correlated patterns of bone destruction with growth rate: geographic lesions with sclerotic or sharp margins grow slowly, while moth-eaten and permeative patterns grow rapidly. Madewell and colleagues refined margin concepts (including changing margination and radiographically occult disease). The 2016 modified Lodwick-Madewell system regroups patterns into grades that better track malignancy risk: grade I usually benign, grade II intermediate, grade III high likelihood of malignancy—while reminding readers that osteomyelitis and eosinophilic granuloma can mimic aggressive grades.
