How this tool works
Ficat and Arlet staged idiopathic femoral head osteonecrosis using radiographs, bone scan, and later MRI, emphasizing the progression from occult ischemia through sclerosis/cysts, subchondral fracture (crescent sign), collapse, and secondary osteoarthritis. The Association Research Circulation Osseous (ARCO) system has been revised (notably 2019) into a streamlined four-stage scheme that incorporates MRI early and subdivides stage III by depression depth. Both remain in clinical use for prognosis and surgical decision-making (core decompression vs arthroplasty).
