ALBI (Albumin-Bilirubin) Grade
ALBI (Johnson 2015)Objective liver-function grading from serum albumin and total bilirubin alone. ALBI = 0.66 x log10(bilirubin in umol/L) - 0.085 x albumin (g/L), banded into grades 1-3 that stratify prognosis and therapy selection in HCC.

How this tool works
The ALBI grade was derived from a large international HCC cohort as an evidence-based, objective alternative to Child-Pugh for assessing liver functional reserve. The score uses only total bilirubin (in umol/L, log10-transformed) and albumin (in g/L): ALBI = (log10 bilirubin x 0.66) + (albumin x -0.085). Scores of -2.60 or less define grade 1, scores above -2.60 up to and including -1.39 define grade 2, and scores above -1.39 define grade 3. ALBI discriminates survival within Child-Pugh class A, predicts outcomes after resection, ablation, TACE and systemic therapy, and is incorporated into the 2022 BCLC model. The calculator accepts bilirubin in mg/dL or umol/L and albumin in g/dL or g/L and converts internally (1 mg/dL bilirubin = 17.1 umol/L; 1 g/dL albumin = 10 g/L), because the formula is strictly defined in SI units.
