How this tool works
Bladder injuries accompany pelvic fractures and blunt lower abdominal trauma. The AAST scale combines wall thickness, rupture size, intraperitoneal versus extraperitoneal location, and involvement of the bladder neck or trigone. Practically, management still hinges more on intraperitoneal versus extraperitoneal location and on complexity (bladder neck, bone fragments, rectal/vaginal injury) than on numeric grade alone: uncomplicated extraperitoneal ruptures often heal with catheter drainage, whereas intraperitoneal ruptures require surgical repair. CT or conventional cystography with adequate distension is required for reliable diagnosis.
