How this tool works
Adrenal injuries are uncommon on trauma CT (about 1–2%) but mark high-energy mechanisms and are associated with increased overall mortality, largely from concurrent injuries. The AAST scale progresses from contusion to cortical-only laceration, laceration into the medulla, greater than 50% parenchymal destruction, and total destruction or vascular avulsion. Most isolated low-grade adrenal hematomas are managed non-operatively. Bilateral severe injury raises concern for acute adrenal insufficiency. Right-sided injuries predominate because of compression against the spine and transmitted venous pressure.
