How this tool works
Urethral injuries accompany pelvic fractures (posterior urethra) and straddle trauma (anterior urethra). The AAST scale is based on retrograde urethrography findings: normal study with blood at the meatus (contusion), elongation without extravasation (stretch), extravasation with bladder filling (partial disruption), extravasation without bladder filling and <2 cm separation (complete), and complete disruption with >2 cm separation or extension into prostate/vagina. Never force a urethral catheter when injury is suspected—obtain a retrograde urethrogram first. Initial management prioritizes urinary drainage (usually suprapubic) with delayed definitive urethroplasty for complete posterior disruptions after pelvic healing.
