How this tool works
The Rotterdam CT score (Maas et al., 2005) was designed to improve outcome prediction over Marshall by scoring individual CT elements additively. Points: basal cisterns normal 0 / compressed 1 / absent 2; midline shift ≤5 mm = 0, >5 mm = 1; epidural mass lesion present = 0, absent = 1 (epidural hematoma is relatively protective); traumatic SAH or IVH absent = 0, present = 1. One point is added to the sum so the final score ranges from 1 to 6. Higher scores predict higher six-month mortality (approximately 0–5% at 1 up to ~61% at 6 in the original adult cohorts). Use on admission CT in moderate–severe TBI alongside clinical factors (GCS, pupils, age).
