How this tool works
The AAST thoracic vascular injury scale ranks injured vessels by physiologic importance. Grade I covers intercostal, internal mammary, bronchial, esophageal arteries/veins and similar; grades progress through azygos/jugular/subclavian veins, carotid/innominate/subclavian arteries, descending aorta and branch pulmonary vessels, then ascending aorta/arch and main pulmonary trunks, ending at uncontained total transection of the thoracic aorta or pulmonary hilum (grade VI). Hemothorax is scored on this scale rather than the lung scale. Management ranges from observation or embolization of small vessels to endovascular or open repair of aortic and great-vessel injuries.
