How this tool works
The AAST scrotum injury scale describes wall injury independent of the testis scale. Grade I is contusion; grades II–III are lacerations stratified by <25% vs >25% of scrotal diameter; grades IV–V are avulsions involving <50% vs >50% of the scrotum. Always evaluate for concurrent testicular, urethral, or cord injury. Simple lacerations without testicular injury can often be closed primarily; major avulsions need debridement and staged reconstruction or grafting while preserving viable gonads when the cord is intact.
