How this tool works
The AAST testis injury scale: Grade I contusion or hematoma; Grade II subclinical laceration of the tunica albuginea; Grade III tunica laceration with <50% parenchymal loss; Grade IV major tunica laceration with ≥50% parenchymal loss; Grade V total testicular destruction or avulsion. Ultrasound helps but can miss rupture—large hematoceles often warrant exploration. Early exploration improves salvage. Conservative care is reasonable for grade I; grade II and higher generally undergo exploration with debridement and tunica closure, partial orchiectomy, or orchiectomy for non-salvageable grade V injuries.
