How this tool works

The Marshall CT classification (1991/1992) stratifies closed TBI on non-contrast head CT using swelling (basal cistern compression and midline shift) and the presence of high- or mixed-density lesions larger than 25 cm³, plus whether any lesion was surgically evacuated. Categories range from Diffuse Injury I (no visible pathology) through IV (shift without large mass), then Evacuated Mass Lesion (V) and Non-Evacuated Mass Lesion (VI). Higher categories correlate with worse outcome and mortality, though the system can be awkward when multiple injury types coexist; the later Rotterdam score addresses some of those limitations. Classification is primarily descriptive/prognostic and does not replace clinical ICP management decisions.
