How this tool works

The AAST lung injury scale ranks contusion extent, simple versus major air leak, intraparenchymal hematoma behavior, and pulmonary vascular disruption up to total uncontained hilar transection. Hemothorax is scored under the thoracic vascular scale, not the lung scale. Advance one grade for multiple injuries up to grade III. Most contusions and simple pneumothoraces are managed non-operatively with support and drainage; central vascular and major hilar injuries require operative or endovascular strategies.
