How this tool works
MELD 3.0 (Kim et al., 2021; adopted for U.S. allocation in July 2023) updates MELD-Na by adding serum albumin and a female-sex coefficient, including bilirubin-sodium and albumin-creatinine interaction terms, and lowering the creatinine ceiling to 3.0 mg/dL. Laboratory values are floored at 1.0 (bilirubin, INR, creatinine); sodium is bounded to 125-137 and albumin to 1.5-3.5. Patients dialyzed at least twice, or receiving 24 hours of CVVHD, in the week before the creatinine measurement are assigned a creatinine of 3.0. Candidates registered at 12-17 years use a variant without the sex term and with a constant of 7.33 instead of 6; the score is not intended below age 12.
