Liver Lesion and LI-RADS Tools
A liver observation is scored in a context: is this a cirrhotic liver, how good is the function, and is the question a category or a treatment stage? These tools answer different parts of that. The lesion category is not the BCLC stage, and neither is a fibrosis blood test.
Educational reference for qualified professionals. Confirm the result against the source cited on the tool page and against local protocol.
The observation
Start here when the report needs a category or a size, not a treatment recommendation.
- LI-RADS v2018 (CT/MRI)Calculator
LI-RADS category for an observation in a high-risk liver.
- Hepatic Steatosis (CT & MRI)Calculator
MRI fat fraction from dual-echo signal, when the question is steatosis rather than a mass.
- Liver Volume (CT/MRI/US) + Expected VolumeCalculator
Liver volume from measurements, including future-liver-remnant style sums.
- Characterisation of an incidental liver lesion (non-cirrhotic liver)Vetting
Which study an incidental liver lesion actually needs.
- Suspected hepatocellular carcinoma in cirrhosisVetting
Workup when the clinical question is hepatocellular carcinoma in a high-risk liver.
Function, fibrosis and stage
These use laboratory values or a treatment framework. They do not assign a LI-RADS category.
- Child-Pugh Score (Child-Turcotte-Pugh)Calculator
Child-Pugh class from bilirubin, albumin, INR, ascites and encephalopathy.
- ALBI (Albumin-Bilirubin) GradeCalculator
ALBI grade from bilirubin and albumin alone.
- FIB-4 (Fibrosis-4) IndexCalculator
FIB-4 fibrosis estimate from age, AST, ALT and platelets.
- APRI (AST-to-Platelet Ratio Index)Calculator
APRI from AST and platelets.
- BCLC Staging & Treatment Strategy (2022 Update)Calculator
BCLC stage and the treatment strategy tied to that stage.
- Milan Criteria / UCSF (Expanded) CriteriaCalculator
Whether tumour burden meets transplant selection criteria.
Questions these pages get
Does LI-RADS replace Child-Pugh or BCLC?
No. LI-RADS categorises an observation. Child-Pugh and ALBI describe liver function. BCLC allocates a treatment stage from tumour burden, function and performance status. A report often needs more than one of them, which is why they are separate tools.
