How this tool works
The mTICI scale grades antegrade reperfusion of the previously occluded target arterial territory on catheter angiography at the end of mechanical thrombectomy. Grade 2 was split so that 2a is <50% and 2b is ≥50% of the target territory; many centers further distinguish 2c (near-complete reperfusion with only slow flow or a few distal cortical emboli) from full 3. Successful reperfusion in trials is usually defined as mTICI 2b–3; functional outcomes are better still with 2c/3 than with 2b alone. The grade is a technical endpoint and must be interpreted with baseline infarct burden, time metrics, and clinical exam.
