How this tool works

ABCD2 stratifies short-term stroke risk after TIA to support triage (expedited outpatient work-up vs admission). Points: age ≥60 (1), BP ≥140/90 at presentation (1), unilateral weakness (2) or speech disturbance without weakness (1), duration ≥60 min (2) or 10–59 min (1), and diabetes (1). Clinical-feature and duration categories are mutually exclusive within each domain. Absolute risks from older cohorts overestimate event rates in modern rapid-access pathways; the score does not replace vascular imaging, ECG/AF detection, or clinical judgment for high-risk mechanisms (e.g. critical carotid stenosis).
