How this tool works

ARWMC was built so the same visual rules work on CT and MRI. Five regions are scored bilaterally: frontal white matter, parieto-occipital white matter, temporal white matter, basal ganglia / deep grey (including thalamus and internal/external capsule areas as defined in the paper), and infratentorial (brainstem and cerebellum). White-matter regions use 0 = none, 1 = focal, 2 = beginning confluence, 3 = diffuse regional involvement. Basal ganglia uses a lesion-count/confluence variant (0 none; 1 single ≥5 mm; 2 multiple focal; 3 confluent). Maximum total is 30. Higher burden associates with gait impairment, cognitive slowing, and vascular risk — complementary to Fazekas and volumetric WMH measures.
