Both testes and epididymes with side-to-side comparison, and spermatic cords
When to use it
- Acute scrotal pain — suspected torsion, epididymo-orchitis or torted appendix.
- Scrotal mass or suspected testicular tumour.
- Trauma.
Technique
- Comparative imaging of both sides in the same window is what makes asymmetric flow interpretable.
Where it goes wrong
- Preserved flow does not exclude torsion, particularly incomplete or intermittent torsion — a clinically convincing history warrants surgical exploration regardless of the scan.
