Builds a standardized PROMISE molecular-imaging TNM (miTNM) stage from PSMA-PET findings. Select miT, miN, and miM (including bone-pattern subtypes) to generate the composite stage string, plain-language interpretation, and management-oriented notes aligned with PROMISE V2 / EAU-EANM reporting.

miT — Local tumor

PROMISE V2 local category. Use miTr for prostate-bed recurrence after RP; use miT2–4 for intact prostate (optionally add sextant map in the report).

miN — Pelvic lymph nodes

miN1 = single pelvic nodal region; miN2 = ≥2 regions. Template stations: internal iliac (II), external iliac (EI), obturator (OB), presacral (PS), other pelvic (OP), with laterality.

miM — Distant metastasis

miM1a = extrapelvic nodes; miM1b = bone (choose pattern); miM1c = visceral/other. Common iliac nodes are distant (miM1a), not pelvic N.

How this tool works

PROMISE (Prostate Cancer Molecular Imaging Standardized Evaluation) defines miTNM categories for whole-body PSMA-ligand PET reporting (Eiber et al. 2018; updated PROMISE V2, Seifert et al. 2023). Local tumor (miT), pelvic nodes (miN0–2 by number of pelvic regions), and distant disease (miM0 / miM1a nodes / miM1b bone with uni|oligo|diss|dmi patterns / miM1c visceral) mirror pathologic TNM while remaining PET-specific. PROMISE V2 also integrates PRIMARY score for the intact gland and a PSMA-expression score (not entered in this calculator).
