How this tool works
The biophysical profile (Manning 1980) combines the nonstress test with four ultrasound-observed variables, each scored 2 when its criterion is met within a 30-minute observation window and 0 otherwise. A total of 8–10 is reassuring and is managed on the routine testing schedule. A score of 6 is equivocal: the test is repeated within 12–24 hours, and delivery is considered at term, with persistent oligohydramnios, or if the repeat score remains equivocal. A score of 4 or less is abnormal and usually prompts delivery once fetal maturity permits; scores of 0–2 call for expedited delivery absent extraordinary circumstances. A reassuring total should not override the significance of a persistently abnormal single component — in particular, oligohydramnios as the sole abnormality still warrants escalation. The modified BPP (NST plus amniotic fluid index only) is a separate screening tool.
